ABSTRACT

As lesões na face decorrentes de mordedura humana estão entre os tipos de trauma causados por violência interpessoal. Injúrias causadas nesta região podem gerar uma série de morbidades como perdas teciduais importantes, infecções, além de cicatrizes irreversíveis quando a ferida é passível de reparo, portanto, os cuidados pós-trauma devem visar à minimização de cura, além da funcionalidade, buscando o máximo de estética possível. Este trabalho tem como objetivo relatar um caso clínico de mordida humana em região de dorso nasal, com avulsão da ponta nasal, atendida no Hospital da Restauração Governador Paulo Guerra, Recife-PE, pelo serviço de Cirurgia Buco-Maxilo-Facial. Procedeu-se com reparo primário e antibioticoterapia profilática, no qual o resultado do tratamento foi considerado satisfatório, boa cicatrização tecidual e sem queixas funcionais e estéticas... Injuries to the face resulting from human bite are among the types of trauma caused by interpersonal violence. The injuries caused in this region, can generate a series of morbidities such as major tissue loss, infection, and irreversible scarring when the wound is repairable, therefore, posttrauma care should aim to minimize healing, in addition to functionality, seeking maximum aesthetics as possible. This paper aims to report a clinical case of human bite in the nasal dorsum region, with avulsion of the nasal tip, attended at the Hospital da Restauração Governador Paulo Guerra, Recife-PE, by the service of Oral and Maxillofacial Surgery. It was proceeded with primary repair and prophylactic antibiotic therapy, in which the result of the treatment was considered satisfactory, good tissue healing and without functional and aesthetic complaints... Las lesiones faciales producidas por mordeduras humanas se encuentran entre los tipos de traumatismos causados por la violencia interpersonal. Las lesiones causadas en esta región pueden generar una serie de morbilidades como la pérdida de tejidos importantes, la infección y la cicatrización irreversible cuando la herida es reparable, por lo tanto, los cuidados posttrauma deben tener como objetivo minimizar la cicatrización, además de la funcionalidad, buscando la máxima estética posible. Este trabajo tiene como objetivo reportar un caso clínico de mordedura humana en la región del dorso nasal, con avulsión de la punta nasal, atendido en el Hospital da Restauração Governador Paulo Guerra, Recife-PE, por el servicio de Cirugía Oral y Maxilofacial. Abstract Introduction Motorcycles are used as a common means of transportation, and motorcycle accidents are responsible for a major portion of trauma injuries. Objectives The purpose of this study was to analyze the patterns of facial injuries in motorcyclists, to evaluate the types of injuries, and to investigate if the accidentrelated factors had any impact on the characteristics of the injuries. Methods This retrospective observational study included 74 patients with maxillofacial injuries following motorcycle-related accidents. Investigated data were divided into four main categories: sociodemographic, accident-related, injury-related, and treatment-related. Results All the patients were males with a mean age (±SD) of 25.03 (±9.986) years. Most accidents (n = 44, 59.4%) occurred in the evening. Most of the patients (n = 40, 54%) were traveling on motorcycle models that had maximum speed of over 120 km/h. Furthermore, 15 patients (18.9%) were under the influence of alcohol during the crashes and only one patient was wearing a helmet. Fractures of the maxillofacial bones were observed in 50 (67.5%) crash victims; 24 of them (48%) had middle third fractures, 11 (22%) had mandibular fractures, and 15 patients (30%) presented with a combination of lower, middle, and upper third fractures. Conclusion Almost all patients were not wearing helmets at the moment of the crash. The most common fractured site was the maxilla. The majority of the patients received surgical treatment. Increased enforcement of safety measures for riders and raising awareness about the dangers of motorcycle crashes are required measures to improve traffic safety and, ultimately, population health.

RESUMEN Fundamento Para el manejo quirúrgico del paciente con deformidad facial resulta vital el uso del examen clínico y radiográfico de cada caso, pues en dependencia de los hallazgos se decide entre las diversas alternativas terapéuticas disponibles en este campo. Objetivo describir el comportamiento imagenológico de las deformidades faciales en pacientes adultos. Métodos se realizó un estudio descriptivo de corte transversal, en 46 pacientes mayores de 18 años, con diagnóstico de deformidad facial, los cuales acudieron al Servicio de Cirugía Maxilofacial, del Hospital Calixto García, en el período comprendido desde noviembre de 2017 a noviembre de 2019. Se analizaron las variables: tipo de tejido, magnitud, y plano en el que se evidenciaron las alteraciones. Se utilizó la prueba exacta de Fisher, con un nivel de confianza al 95 %, para determinar las relaciones analíticas entre las variables; así como los índices tau b de Kendall y el Rho de Spearman. Resultados el 45,7 % de las deformidades se presentó en ambos planos; el 50 % de ellas con magnitudes entre 11-30 mm; y en el 41,3 % de los pacientes afectaron al tejido blando. Las correlaciones entre la distribución de pacientes y los conjuntos de variables plano/tipo de tejido y plano/magnitud resultaron negativas débiles; mientras que tipo de tejido/magnitud presentó una correlación positiva moderada muy significativa, con valores de significación entre 0,001 a 0,003. Conclusión predominaron las deformidades de tejido blando, en ambos planos y con magnitudes entre 11-30 mm. Las variables imagenológicas estudiadas mostraron un comportamiento interdependiente.


ABSTRACT Background For the surgical management of the patient with facial deformity, the use of the clinical and radiographic examination of each case is vital, since depending on the findings, a decision is made between the various therapeutic alternatives available in this field. Objective to describe the imaging behavior of facial deformities in adult patients. Methods a descriptive cross-sectional study was carried out in 46 patients older than 18 years old, with a diagnosis of facial deformity, who attended the Maxillofacial Surgery Service, Calixto García Hospital, from November 2017 to November 2019. The analyzed variables were: type of tissue, magnitude, and plane in which the alterations were evidenced. Fisher's exact test was used, with a confidence level of 95 %, to determine the analytical relationships between the variables; as well as Kendall's tau b and Spearman's Rho rates. Results The 45.7 % of the deformities presented in both planes; 50 % of them with magnitudes between 11-30 mm; and in 41.3 % of the patients they affected the soft tissue. The correlations between the distribution of patients and the sets of variables plane/tissue type and plane/magnitude were weakly negative; while tissue type/magnitude presented a very significant moderate positive correlation, with significance values between 0.001 and 0.003. Conclusion soft tissue deformities predominated, in both planes and with magnitudes between 11-30 mm. The imaging variables studied showed an interdependent behavior.

RESUMEN Fundamento Las fracturas del complejo maxilofacial constituyen cerca de la mitad de todas las fracturas, y en un gran número de pacientes se asocian a otras fracturas y lesiones. La etiología es variada, y exigen diagnóstico y tratamiento rápidos, precisos y oportunos. Resulta relevante el estudio de esta entidad, para conocer sus características como parte de la atención de urgencias; así como sus implicaciones clínica y social. Objetivo describir el comportamiento de las fracturas maxilofaciales en pacientes atendidos en el servicio de Cirugía Maxilofacial del Princess Marina Hospital, de Gaborone, Botswana. Métodos se realizó un estudio descriptivo y retrospectivo, en el servicio de Cirugía Maxilofacial del Princess Marina Hospital, en Gaborone, Botswana, durante el período de junio de 2018 a junio de 2020. Fueron incluidos todos los pacientes (N=423) con diagnóstico de trauma maxilofacial, y por consiguiente, de algún tipo de fractura de esta parte del cuerpo. Para la presentación de la información se utilizaron tablas de frecuencias absolutas y relativas. Resultados se atendieron 423 pacientes con fracturas maxilofaciales, entre los cuales predominó el sexo masculino. El grupo de edades más afectado fue el de 26-35 años; la violencia interpersonal, la causa más frecuente; y las heridas y fracturas de cráneo, las lesiones asociadas más comunes. Conclusión Se observó similitud de los resultados respecto a otros estudios publicados. La violencia interpersonal superó a los accidentes del tránsito como la causa más frecuente de fracturas maxilofaciales.


ABSTRACT Background Fractures of the maxillofacial complex constitute about half of all fractures, and in a large number of patients they are associated with other fractures and injuries. The etiology is varied, and requires fast, accurate, and timely diagnosis and treatment. The study of this entity is relevant to know its characteristics as part of emergency care; as well as its clinical and social implications. Objective to describe the behavior of maxillofacial fractures in patients treated at the Maxillofacial Surgery service of Princess Marina Hospital, Gaborone, Botswana. Methods a descriptive and retrospective study was carried out in the Maxillofacial Surgery service of the Princess Marina Hospital, in Gaborone, Botswana, from June 2018 to June 2020. All patients (N = 423) with diagnosis were included of maxillofacial trauma, and consequently, of some type of fracture of this part of the body. Absolute and relative frequency tables were used to show the information. Results 423 patients with maxillofacial fractures were attended, among which the male sex predominated. The age group most affected was 26-35 years old; interpersonal violence, the most frequent cause; and skull injuries and fractures, the most common associated injuries. Conclusion Results were similar to other published studies. Interpersonal violence exceeded traffic accidents as the most common cause of maxillofacial fractures.

ABSTRACT

Oral myiasis is a relatively rare and uncommon clinical condition arising from the invasion of body tissues or cavities of living animals or humans by maggots or larvae of certain species of flies. This condition may be observed in suppurative oral lesions, alcoholism, senility and also maintaining poor oral hygiene by patients during orthodontic treatment, invasion and growth of larvae during and after maxillofacial injuries. The current article presents with management of oral myiasis during orthodontic treatment, and in oral and maxillofacial injuries.

ABSTRACT

ABSTRACT Injuries caused by firearm projectiles in the head and neck region generally imply severe morbidity and mortality, representing an important public health problem. These injuries can be classified as: penetrating, perforating and avulsive, and the latter is the most worrisome, because they are caused by high-velocity projectiles. Low-velocity projectiles commonly cause penetrating injuries, sometimes remaining lodged in tissues, and their removal may or may not be indicated, depending on factors such as location and severity of the lesion. This paper presents the report of three clinical cases that address injuries caused by low-velocity firearm projectiles, in which they had the indication of this artifact removal, addressing the pre, trans and post-surgery procedures. And we point out that full tactical, technical and theoretical domains are required from the Oral-Maxillofacial Surgeons for being successful in the planning and success in the treatment of these lesions.


RESUMO As lesões provocadas por projetil de arma de fogo em região de cabeça e pescoço implicam, geralmente, em morbidade e mortalidades severas representando um importante problema de saúde pública. Esses ferimentos podem ser classificados como: penetrantes, perfurantes e avulsivos, sendo este último o mais preocupante, por serem ocasionados por projéteis de alta velocidade. Os projéteis de baixa velocidade causam comumente ferimentos penetrantes, permanecendo por vezes alojados nos tecidos, podendo ou não, ser indicada a sua remoção, a depender de fatores como localização e gravidade da lesão. O presente artigo traz o relato de três casos clínicos que abordam lesões por projeteis de arma de fogo de baixa velocidade, nas quais tiveram a indicação de remoção do referido artefato, abordando-se as condutas pré, trans e pós-operatórias. Observando-se que é requerido dos Cirurgiões Buco-Maxilo-Faciais, plenos domínios tático, técnico e teórico, para se obter êxito no planejamento e sucesso no tratamento dessas lesões.

RESUMEN: La atención de urgencia dental se desenvuelve en un contexto de estrés que afecta el comportamiento cardiovascular de los pacientes, expresado como cambios en Frecuencia Cardiaca y Presión Arterial. Se espera que posterior a la atención, estos parámetros varíen conforme se controla el cuadro clínico o se presenten complicaciones. Un estudio observacional retrospectivo recopiló los registros de Frecuencia Cardiaca y Presión Arterial de pacientes atendidos en la urgencia dental del Complejo Asistencial Barros Luco en dos ocasiones, estando la segunda consulta vinculada al primer ingreso, tanto como control post tratamiento o complicaciones derivadas. Datos fueron registrados en Microsoft Excel™ y analizados en SciStat™. 466 pacientes consultaron en dos ocasiones, concentrados entre tercera y quinta década. Los mayores registros de Presión Arterial fueron reportados en controles post atención de Pericoronaritis, infecciones de espacios anatómicos faciales y en complicaciones post tratamiento por Pulpitis, TDA e Infección de espacios anatómicos faciales. En segundas consultas por control de Pulpitis, Alveolitis, TDA, Fracturas faciales y Hemorragias post exodoncia hubo disminución. Frecuencia Cardiaca fue el parámetro con mayor variación existiendo disminución en controles y complicaciones entre consultas. El papel exacto del dolor en la respuesta cardiovascular no está bien delimitado, la interpretación de las investigaciones destinadas a estudiar esto resulta difícil, pues estímulos nociceptivos son sólo uno de los activadores de la respuesta sistémica al estrés. En el postoperatorio el dolor juega un papel importante estudiado a 48 horas, más no, en mediano y largo plazo. Existen variaciones estadísticamente significativas en algunos parámetros estudiados, los cuales pueden presentar significancia clínica a considerar en pacientes hipertensos o cardiópatas. Son necesarios estudios sobre la incidencia particular que tienen los procedimientos odontológicos en la expresión de otros marcadores del estado sistémico.


ABSTRACT: Emergency dental care develops in a context of stress that affects the cardiovascular behavior of patients, expressed as changes in heart rate and blood pressure. After care, these parameters are expected to vary as the clinical picture is controlled or complications occur. A retrospective observational study compiled the heart rate and blood pressure records of patients seen in the dental emergency of the Barros Luco Care Complex on two occasions, the second consultation being linked to the first admission, as well as post-treatment control or derived complications. Data was recorded in Microsoft Excel ™ and analyzed in SciStat ™. 466 patients consulted on two occasions, concentrated between the third and fifth decade. Higher Blood Pressure records were reported in post-care controls for Pericoronitis, infections of facial anatomical spaces and in post-treatment complications due to Pulpitis, ADD and Infection of facial anatomical spaces. In second consultations for control of Pulpitis, Alveolitis, ADT, facial fractures and post- extraction hemorrhages there was a decrease. Heart rate was the parameter with the greatest variation, with a decrease in controls and complications between consultations. The exact role of pain in the cardiovascular response is not well defined, the interpretation of the research aimed at studying this is difficult, since nociceptive stimuli are only one of the activators of the systemic response to stress. In the postoperative period, pain plays an important role, studied at 48 hours, but not in the medium and long term. There are statistically significant variations in some parameters studied which may present clinical significance to be considered in hypertensive or heart disease patients. Studies are needed on the particular incidence that dental procedures have on the expression of other markers of the systemic state.

Background: This retrospective study was conducted to identify the epidemiological profile and treatment modalities linked to the maxillofacial trauma (MFT) managed in the Maxillofacial Surgery Departments of seven hospital centers in Antioquia, Colombia. Material and Methods: Clinical records with specific attention to sociodemographic characteristics, mechanisms of injury, type of MFT, location of injuries, and treatment modalities of MFT were collected from January to December 2017. Descriptive analyses using Pearson's chi-square tests were performed. Results: A total of 1356 records were retrieved. Males were significantly more affected, with a male-to-female ratio of 3.85:1. The most susceptible age group involved was young adults (18 to 40 years). A low percentage of alcohol (9.3%) and drugs consumption (2.5%) was recorded. Most common causes of MFT were road traffic accidents (RTA), falls, and interpersonal violence (IPV). Most injuries involved both soft and hard tissues followed by hard tissues and isolated open soft tissue injuries. Among fractures, the middle third was the most commonly affected site and the utmost method of treatment was open reduction and internal fixation. Conclusion: Within the limitations of the evidence available, this study has demonstrated that the gender, age stratum, and etiological factors, such as RTA, falls, and IPV, but no alcohol and/or psychoactive substances consumption, may have a significant influence on the prevalence, patterns, and treatment modalities of MFT in this sample population.


Antecedentes: Este estudio retrospectivo se realizó para identificar el perfil epidemiológico y las modalidades de tratamiento vinculados al Trauma Maxilofacial (TMF) atendido en los Servicios de Cirugía Maxilofacial de siete centros hospitalarios de Antioquia, Colombia. Material y Métodos: Se recopilaron historias clínicas con atención específica a las características sociodemográficas, mecanismos de lesión, tipo de TMF, ubicación de las lesiones y modalidades de tratamiento de TMF de enero a diciembre de 2017. Análisis descriptivos utilizando el chi-cuadrado de Pearson fueron realizados. Resultados: Se recuperaron un total de 1356 registros. Los hombres se vieron significativamente más afectados, con una relación hombre-mujer de 3,85:1. El grupo de edad más susceptible involucrado fue el de los adultos jóvenes (18 a 40 años). Se registró un bajo porcentaje de consumo de alcohol (9,3%) y drogas (2,5%). Las causas más comunes de TMF fueron los accidentes de tránsito (ADT), las caídas y la violencia interpersonal (VIP). La mayoría de las lesiones involucraron tejidos blandos y duros, seguidas de tejidos duros y lesiones abiertas aisladas de tejidos blandos. Entre las fracturas, el tercio medio fue el sitio afectado con mayor frecuencia y el método de tratamiento más utilizado fue la reducción abierta y la fijación interna. Conclusion: Dentro de las limitaciones de la evidencia disponible, este estudio ha demostrado que el género, el estrato etario y los factores etiológicos, como ADT, caídas y VIP , pero no el consumo de alcohol y/o sustancias psicoactivas, pueden tener una influencia significativa en la la prevalencia, los patrones y las modalidades de tratamiento de TMF en esta poblaión muestral.

ABSTRACT

RESUMEN Fundamento: el uso de protectores bucales durante la práctica deportiva es fundamental como medida preventiva de las lesiones por traumatismos dentomaxilofaciales en la adolescencia. Objetivo: explicar la importancia del uso de los protectores bucales al tener en cuenta sus características, tipos y funciones. Métodos: se realizó una revisión de la literatura procedente de bases de datos como CUMED, LILACS, MEDLINE, SCOPUS y PUBMED, sitios web, libros y revistas en Internet, de artículos publicados durante los últimos 20 años, sobre los tipos de protectores bucales y su importancia para la prevención de las lesiones por traumatismos dentomaxilofaciales. Se encontraron un total de 60 trabajos, de los cuales se seleccionaron 30, se tuvo en cuenta su actualidad, relevancia y pertinencia sobre el tema. Resultados: el protector bucal es un elemento básico en la prevención de las lesiones dentomaxilofaciales en la adolescencia. En la actualidad existen tres tipos: prefabricados, adaptables e individualizados. Su propósito es distribuir la fuerza de los impactos recibidos en una situación de contacto, disminuyendo la posibilidad de sobrecargas puntuales. Conclusiones: los protectores bucales individualizados son los más efectivos para la prevención de las lesiones traumáticas dentomaxilofaciales y su función más importante es estabilizar la mandíbula y minimizar el riesgo de la contusión y las lesiones cerebrales. Los estomatólogos recomiendan su uso obligatorio en la práctica de cualquier deporte, sea aficionado o profesional.


ABSTRACT Background: the use of mouth guards during sports practice is essential as a preventive measure for dent-maxillofacial trauma injuries in adolescence. Objective: to explain the importance of the use of mouth guards taking into account their characteristics, types and functions. Methods: a revision of the literature was carried out from databases such as CUMED, LILACS, MEDLINE, SCOPUS and PUBMED, websites, books and magazines on the Internet, of articles published during the last 20 years, on the types of mouth guards and their importance for the prevention of dent-maxillofacial trauma injuries. Sixty works were found, of which 30 were selected taking into account their actuality and relevance to the topic. Results: the mouth guard is a basic element in the prevention of dent-maxillofacial lesions in adolescence. Currently, there are three types: prefabricated, adaptable and individualized. Its purpose is to distribute the force of the impacts received in a contact situation, reducing the possibility of specific overloads. Conclusions: individualized mouth guards are the most effective for traumatic dent-maxillofacial injuries prevention and their most important function is to stabilize the jaw and minimize the risk of contusion and brain injury. Dentists recommend its mandatory use in the practice of any sport, being it amateur or professional.

A violência interpessoal tem se configurado como um grave problema social e de saúde pública, em que lesões na região bucomaxilofacial são importantes consequências. O objetivo desse estudo foi analisar o perfil das vítimas de violência interpessoal que tiveram lesões na região maxilofacial e foram atendidas no hospital da Restauração em Recife, Pernambuco-Brasil durante o ano de 2017. Foi realizada uma pesquisa retrospectiva e exploratória baseada nas informações contidas nos prontuários do hospital e em seguida foi utilizado o software Statistical Package for the Social Sciences (SPSS) para análise das medidas descritivas. No período de um ano foram encontrados 455 prontuários, de forma que 87,9% corresponderam ao sexo masculino e 12,1% ao feminino. Do total, 34,9% tinham idade entre 20-29 anos e 43,3% foram vítimas de força corporal/espancamento. O tipo de trauma mais frequente foi fratura dos ossos maxilofaciais (45,3%). Outrossim, o dia e horário mais comum de atendimento foi o domingo (23,1%) no período noturno (58,2%). Homens jovens com traumas dos ossos maxilofaciais foi o principal perfil encontrado no estudo, sendo necessário o fortalecimento de políticas públicas a fim de evitar o aumento de casos nessa população... Interpersonal violence has been configured as a serious social and public health problem in which injuries on the maxillofacial region are important consequences. This study aimed to analyze the profile of interpersonal violence victims who had maxillofacial injuries and were treated at Hospital Da Restauração in Recife, Pernambuco-Brazil during 2017. A retrospective and exploratory research was conducted based on information from the medical records of the hospital and then the Statistical Package for the Social Sciences (SPSS) Software was used for descriptive measures analysis. In the period of one year, 455 medical records were found, in which 87,9% corresponded to male and 12,1% to female. Of the total, 34,9% were aged between 20-29 years old and 43,3% were victims of physical strength/beating. The most frequent type of trauma was fracture of the maxillofacial bones (45,3%). Otherwise, the most common day and time of treatment at the hospital was on Sundays (23,1%) and in the evening (58,2%). Young men with trauma on the maxillofacial bones was the main profile found in this study, requiring the strengthening of public policies to avoid the increase in cases in this population... (AU)

Um dos principais atendimentos feitos pelos serviços de urgência e emergência são os traumas faciais, responsáveis por grandes impactos na vida dos pacientes por causarem morbidades e superlotação em hospitais. Desta forma o presente estudo teve como objetivo realizar um levantamento dos atendimentos de Cirurgia e Traumatologia buco-maxilofaciais em um hospital regional. Foi realizado um levantamento retrospectivo analisando 1333 prontuários dos pacientes atendidos no serviço de Cirurgia e Traumatologia Buco-Maxilo-Facial do Hospital Regional de Presidente Dutra - Maranhão - Brasil, independente da etiologia para o atendimento, no período de março de 2015 a agosto de 2018. A amostra foi constituída por 1333 prontuários, o gênero masculino foi o mais acometido (n=1099) (82%); a faixa etária entre 21 a 30 anos foi a mais prevalente em ambos os gêneros; os acidentes de trânsito foram a principal causa dos traumas faciais, sendo a mandíbula a região mais afetada com 401 casos (30,1%); o procedimento cirúrgico mais realizado foram as osteossínteses, totalizando 57,4%, e o Tempo de internação médio foi de 1 a 5 dias. A principal etiologia das cirurgias foi fratura, devido a acidentes automobilísticos ocasionado pela falta de aplicação e fiscalização das leis de trânsito, contribuindo assim, para o aumento da morbimortalidade e dos custos hospitalares... Introduction: One of the main services provided by emergency and emergency services is facial trauma, responsible for great aesthetic, functional and emotional impacts on individuals lives, as well as causing morbidity and overcrowding in hospitals. In this way, the present study aimed to perform a survey of the operations of buco-maxillofacial Surgery and Traumatology in a large regional hospital in the interior of Maranhão. Methods: a retrospective survey was carried out, analyzing the patients medical records at the Department of Oral and Maxillofacial Surgery and Traumatology of the Presidente Dutra-MA Regional Hospital, regardless of the etiology for care, in the period from March 2015 to August 2018. Results: The sample consisted of 1333 medical records, the male gender was the most affected (n = 1099) (82%); the age group between 21 and 30 years was the most prevalent in both genders; traffic accidents were the main cause of facial trauma, with the jaw being the most affected region with 401 cases (30.1%); the most frequent surgical procedure was osteosynthesis, totaling 57.4% and the mean length of hospital stay was 1 to 5 days. Conclusão: The main etiology of the surgeries was a fracture, due to automobile accidents caused by the lack of application and inspection traffic laws, thus contributing to the increase of morbidity and mortality in hospital costs... Fraturas faciais geralmente resultam traumas, e podem ocorrer de forma isolada ou concomitante a outras lesões. O trauma na região facial resulta em danos não só em tecido ósseo, mas também em tecido mole e dentição, causando um prejuízo na função e na estética do paciente. Os acidentes automotores permanecem sendo uma das causas mais significativas de traumas faciais. As fraturas do terço médio da face incluem a maxila, zigoma e o complexo naso órbito etimoidal; podem ser classificadas em: fraturas Le Fort I, II ou III, fraturas do complexo zigomático maxilar, fraturas de arco zigomático ou fraturas naso órbito etimoidais. Elas podem ser unilaterais ou bilaterais, simétricas ou assimétricas. Este estudo objetiva-se em relatar o caso clínico de um paciente do sexo masculino, 31 anos, vítima de acidente automobilístico (carro/ anteparo). O paciente foi diagnosticado com fratura do tipo Le Fort II e nasal onde foi realizado o tratamento de redução e fixação interna rígida das fraturas de maxila e redução incruenta de fratura nasal, resultando em melhora da condição do paciente. Facial fractures usually result from trauma, and may occur in isolation or concomitantly with other injuries. Trauma in the facial region results in damage not only to bone tissue but also to soft tissue and dental elements, which causes a loss in the patient's function and esthetics. Automotive accidents remain one of the most significant causes of facial trauma. Fractures of the middle third of the face include maxilla, zygoma and the nasoorbitoethmoid; may be classified as: Le Fort I, II or III fractures, maxillary zygomatic complex fractures, zygomatic arch fractures, or nasoorbitoethmoid. They can be unilateral or bilateral, symmetrical or asymmetric. The present study aims to report the clinical case of a 31 years, male patient, victim of an automobile accident (car/fixed shield). The patient was diagnosed with le Fort II and nasal fracture, where the treatment of reduction and rigid internal fixation of maxilla fractures and reduction of nasal fracture were performed, resulting in improvement of the patient's condiction. Le Fort fractures are predominantly caused by high energy collisions. Therefore the treatment is essential for the return of the function, aesthetics and increase of the patient's self-esteem... RESUMEN: Desde la aparición del SARS-CoV-2 se ha establecido, en la mayoría de los países afectados por esta pandemia, la suspensión de toda cirugía electiva, centrando la atención solo a urgencias. Son varios los estudios que mencionan las recomendaciones necesarias, sin embargo, hasta el minuto existe poca evidencia de la cantidad y tipo de cirugías realizadas por los servicios de Cirugía Maxilofacial. El objetivo del presente trabajo es dar a conocer la epidemiología de los pacientes tratados por el equipo de cirugía maxilofacial en el Hospital Barros Luco Trudeau de Santiago, Chile. Se realizó un estudio de corte transversal entre Marzo y Agosto del 2020, de todos los paciente atendidos por la unidad de Cirugía Maxilofacial del Hospital. Se analizó datos epidemiológicos como también el tipo de patología, la etiología, días de estadía hospitalaria y días de espera para cirugía. La muestra estuvo constituida por 41 pacientes, 20 % mujeres y 80 % hombres. El rango etario fue de 17 a 92 años, con un promedio de 40,39 años (DE 17,67). En al diagnóstico, el 61 % correspondió a patología traumática y el 39 % a patología infecciosa. El trauma más frecuente fue la fractura mandibular parasinfisiaria (23 %) y la infección más frecuente, la infección secundaria a quistes y tumores (25 %). A pesar del periodo de pandemia con las restricciones asociadas a la cuarentena, la muestra se comporta similar a estudios previamente reportados tanto en la distribución por sexo y edad, así como en el tipo el trauma.


ABSTRACT: Since the appearance of SARS-CoV-2, the suspension of all elective surgery has been established in most of the countries affected by this pandemic, focusing only on emergency. There are several studies that mention the necessary recommendations, however, until now there is little evidence of the amount and type of surgeries performed by the different Maxillofacial Surgery services. The objective of this study is to present the epidemiology of the patients treated by the maxillofacial surgery team at the Barros Luco Trudeau Hospital of Santiago, Chile. Material and method: A cross-sectional study was carried out between April and August 2020, of all the patients treated by the Maxillofacial Surgery unit of the Hospital. Epidemiological data were analyzed as well as the type of pathology, the etiology, days of hospital stay and days of waiting for surgery. Results: The sample consisted of 41 patients, 20 % women and 80 % men. The age range was from 17 to 92 years, with an average of 40.39 years (SD 17.67). At diagnosis, 61 % corresponded to traumatic pathology and 39 % to infectious pathology. The most frequent trauma was the parasymphyseal mandibular fracture (23 %) and the most frequent infection, infection secondary to cysts and tumors (25 %). Despite the pandemic period with the restrictions associated with quarantine, the sample behaves similar to previously reported studies, both in the distribution by sex and age, as well as in the type of trauma.

ABSTRACT

Leopard attacks on humans are reported most often from the Indian subcontinent. The bite wounds are complex injuries infected with polymicrobial inoculum and may present as punctures, abrasions, lacerations or avulsions. The presentation and acceptable treatment of these injuries vary according to the wound. We hereby describe the clinical presentation and treatment of a male victim with leopard bite injuries on the head and neck region. As bite injuries are commonly found on and around the face, maxillofacial surgeons should be familiar with the therapy. Through thorough clinical and radiological examination, it is essential to prevent missing any hidden injuries, which can easily turn lethal. To benefit the rural population, more health facilities need to be established in remote areas.

ABSTRACT Objective: To assess the influence of oral and maxillofacial trauma on the development of Post-Traumatic Stress Disorder (PTSD) and to determine the efficiency of the Impact of Event Scale-Revised (IES-R) as a diagnostic tool for detecting PTSD in patients with Oral and Maxillofacial injuries. Material and Methods: PTSD was assessed one month postoperatively by the diagnostic instrument, IES-R, to arrive at a provisional diagnosis. A structured clinician-administered PTSD Scale then assessed the patients for the Diagnostic and Statistical Manual of Mental Disorders-5th edition (CAPS-5) to establish a final diagnosis. The assessment of the severity of PTSD was done based on various types of oral and maxillofacial injuries. Results: The IES-R scale provisionally diagnosed 54 subjects with PTSD, out of which 42 were diagnosed to have PTSD by the CAPS-5 scale. Subjects with injuries involving the 'orbital complex,' those presenting with a perceptible scar in the maxillofacial region and with multiple avulsed/ luxated anterior teeth, showed a higher affinity to develop PTSD, and this was statistically significant. Conclusion: Higher levels of PTSD in patients with injuries to the maxillofacial region warrants correct diagnosis and detection, and hence the maxillofacial surgeon plays a vital role in this regard. The IES-R is a useful diagnostic tool to detect PTSD early.

Abstract The aims of this investigation were to describe the profile of men and women victims of violence and identify factors associated with the severity of facial trauma. A retrospective study was carried out from 762 records of victims attended at the Institute of Legal Medicine and Dentistry located in a metropolitan region of Northeastern Brazil. The dependent variable was type of facial trauma suffered by victims. Independent variables were the sociodemographic characteristics of victims, characteristics of aggressors and circumstances of violence. Descriptive, bivariate (c2 test) and multivariate statistics were made through logistic regression. Level of significance was set at p < 0.05. The mean age of victims was 29.78 years (SD=13.33). Based on the final regression model, male subjects [odds ratio (OR)=2.22, 95% CI=1.08-4.57, p=0.030], assaulted by other male subjects (OR=4.88; 95% CI=1.12-21.26; p=0.035) through instrument (OR=6.67; 95% CI=2.85-15.60; p<0,001) or mixed aggressions (OR=4.34; 95% CI=1.44-13.02; p=0.009) were more likely to exhibit facial bone fractures or dentoalveolar fractures. The findings highlight that men and women present important victimization differentials in relation to interpersonal violence and facial trauma. Victim's gender, aggressor's gender and mechanism of aggression may exert influence on facial trauma patterns.


Resumo Os objetivos desta investigação foram descrever o perfil de homens e mulheres vítimas de violência e identificar fatores associados à gravidade do trauma facial. Foi realizado um estudo retrospectivo de 762 prontuários de vítimas atendidas no Instituto de Medicina Legal e Odontologia de uma região metropolitana do Nordeste do Brasil. A variável dependente foi o tipo de trauma facial sofrido pelas vítimas. Variáveis ​​independentes foram as características sociodemográficas das vítimas, características dos agressores e circunstâncias de violência. Estatísticas descritivas, bivariadas (teste c2) e multivariadas foram feitas por meio de regressão logística. O nível de significância foi fixado em p<0,05. A idade média das vítimas foi de 29,78 anos (DP =13,33). Com base no modelo de regressão final, os indivíduos do sexo masculino [odds ratio (OR)=2,22, IC 95%=1,08-4,57, p=0,030], agredidos por outros sujeitos do sexo masculino (OR=4,88; IC 95%=1,12-21,26; p=0,035) por meio de instrumentos (OR=6,67; IC 95%=2,85-15,60; p<0,001) ou agressões mistas (OR=4,34; IC 95%=1,44-13,02; p=0,009) foram mais propensos a apresentar fraturas de ossos faciais ou fraturas dentoalveolares. Os achados apontam que homens e mulheres apresentam importantes diferenciais de vitimização em relação à violência interpessoal e trauma facial. O gênero da vítima, o gênero do agressor e o mecanismo de agressão podem exercer influência sobre os padrões de trauma facial.

ABSTRACT

Abstract Dog bites cause complex craniomaxillofacial injuries. Considering that the bite injuries are repeatedly situated on the face, dentist requires being well known with the therapy of animal bites. A three-year-old male child was attended as a victim of a Pit Bull terrier biting. The patient had multiple wounds at the level of the upper eyelid of the right eyeball, left hemisphere with the involvement of the auricular pavilion, wounds with multiple detachments on face and scalp, several abrasions and ecchymoses in the bilateral infraorbital and mouth region. The clinical intervention was completed in the first twenty-four hours after injury. The affected zones were cleaned and irrigated with saline solution, and the wounds were sutured in layers. Tetanus and anti-rabies vaccination, and antibiotic and analgesic were administered. On the fifth postoperative day, the stitches were removed, and the patient was discharged from the hospital. One week after the trauma, the child showed adequate wound curing absent of aggravation. Satisfactory aesthetic and functional results were achieved. Besides, in this case, the most relevant aspects of the management of dogs bites are presented.


Resumen Las mordeduras de perros causan lesiones craneomaxilofaciales complejas. Teniendo en cuenta que las lesiones por mordedura están situadas repetidamente en la cara, el odontólogo debe conocer la terapia que se debe realizar en el caso de mordeduras de animales. Un niño de tres años fue atendido como víctima de una mordedura de Pit Bull terrier. El paciente tenía múltiples heridas a nivel del párpado superior del globo ocular derecho, hemisferio izquierdo con afectación del pabellón auricular, heridas con múltiples desprendimientos en la cara y el cuero cabelludo, varias abrasiones y equimosis en la región bilateral infraorbitaria y bucal. La intervención clínica se completó en las primeras veinticuatro horas después de la lesión. Las zonas afectadas se limpiaron e irrigaron con solución salina, y las heridas se suturaron en capas. Se administraron vacuna antitetánica y antirrábica, antibióticos y analgésicos. En el quinto día postoperatorio, se retiraron los puntos de sutura y el paciente fue dado de alta del hospital. Una semana después del trauma, el niño mostró curación adecuada de la herida sin agravación. Se lograron resultados estéticos y funcionales satisfactorios. Además, en este caso, se presentan los aspectos más relevantes del manejo de las mordeduras de perros.


Resumo Mordidas de cães causam lesões craniomaxilofaciais complexas. Considerando que as lesões por mordidas são repetidamente situadas na face, um cirurgião oral e maxilofacial exige ser bem conhecido com a terapia de mordidas de animais. Um menino de três anos foi vítima de uma picada de um terrier de Pit Bull. O paciente apresentava múltiplas feridas no nível da pálpebra superior do globo ocular direito, hemisfério esquerdo com envolvimento do pavilhão auricular, feridas com múltiplos descolamentos na face e couro cabeludo, várias abrasões e equimoses na região infraorbital e boca bilateral. A intervenção clínica foi concluída nas primeiras vinte e quatro horas após a lesão. As zonas afetadas foram limpas e irrigadas com solução salina e as feridas foram suturadas em camadas. Vacinação antitetânica e anti-rábica e antibióticos e analgésicos foram administrados. No quinto dia de pós-operatório, os pontos foram retirados e a paciente recebeu alta do hospital. Uma semana após o trauma, a criança apresentou cura adequada da ferida, sem agravamento. Resultados estéticos e funcionais satisfatórios foram alcançados. Além disso, neste caso, são apresentados os aspectos mais relevantes no manejo das mordidas de cães.

Aim: Quantitative evaluation of prognostic correspondence between initial maxillofacial traumatic injury assessed by facial injury severity score and maxillofacial injury severity score, treatment cost and duration of hospitalization among Ukrainian patients. Methods: Design of present study was retrospective and based on the medical data of patients hospitalized with signs of maxillofacial trauma. Quantitative assessment of maxillofacial trauma was held with the use of facial injury severity score (FISS) and maxillofacial injury severity score (MFISS). Average treatment cost and hospitalization duration were used as coordinative criteria for economical treatment-related burden verification. Results: Levels of correlation between FISS, treatment charges and hospitalization duration were r=0.69 (р<0.05) and r=0.67 (р<0.05) respectively, while analogical correlations for MFISS were 0.74 (р<0.05) and 0.69 respectively (р<0.05). Statistical correspondence between FISS and MFISS scores among study sample reached r=0.71 (р<0.05). Cases with milder maxillofacial trauma types, characterized with initial lower levels of FISS and MFISS scores, demonstrated greater degree of FISS-to-MFISS inter-relation compare to cases with severe maxillofacial trauma. Conclusion:Even though FISS and MFISS scores both demonstrated reliable levels of correlation with hospitalization duration and cost of dental rehabilitation after maxillofacial trauma injury, but MFISS approach characterized by prognostically greater level of statistical relationship with economically related treatment derivates. Moreover, differentiation capabilities of MFISS is relative greater than FISS, since independent grading of separate functional disabilities becomes possible

Abstract Objective: To identify the prevalence of maxillofacial injuries resulting from interpersonal violence in Brazilian children and adolescents. Material and Methods: This is a retrospective study conducted at a Center of Forensic Medicine and Forensic Dentistry with a sample of 335 medical reports of victims aged up to 19 years. Variables involved sex and age group, perpetrator, occurrence of the event, characterization of injuries, presence of maxillofacial injuries, type of tissue involvement, and injuries in the oral cavity. Data were analyzed using descriptive, and the Chi-square was used for categorical data. Results: Most victims were female (60.3%) and aged 15-19 years (57.6%). Aggressions occurred at home (50.6%), in the evening (39.5%) and involved perpetrators known to the victim (91.5%). Most victims had multiple injuries (75.8%), involving up to three regions of the body (93.4%). Injuries with blunt objects were the most frequent (86.2%). The prevalence of maxillofacial injuries was 36.7%, with low involvement of the oral cavity (8.4%). A significant association between the presence of injuries on the face and variables "perpetrator" (p=0.015) and "number of injuries" (p=0.006) was observed. Conclusion: Female adolescents were the main victims of physical violence, with repercussions in different regions of the body. The prevalence of maxillofacial injuries was high, although with little involvement of oral cavity structures.

RESUMEN Las fracturas faciales son poco frecuentes en los niños, motivo por el cual se presenta el caso de una niña de 10 años de edad, de color de piel mestiza, procedencia urbana, con antecedentes de salud, la que, tras una caída de aproximadamente 2,5 m de altura, sufrió un severo traumatismo dentoalveolar anterosuperior y anteroinferior que le ocasionó fractura dentoalveolar en el sector anterosuperior de 12 a 22, luxación de 12, 11 y 22 y avulsión de 21(el cual pudo ser recuperado en el lugar del trauma) y fractura dentoalveolar en el sector anteroinferior de 32 a 42. Fue recibida en el cuerpo de guardia del Hospital Pediátrico Universitario Paquito González Cueto y atendida en el salón de Cirugía Maxilofacial de este hospital. Se aplicó tratamiento quirúrgico para heridas de partes blandas realizando sutura, reducción y estabilización de la fractura en el sector anterosuperior mediante férula de Erich y en el sector anteroinferior mediante osteosíntesis con alambre en ocho. Se reimplantó 21 y se fijó a la férula mediante osteosíntesis con alambre. Se indicó tratamiento con dieta blanda y líquida, analgésicos, antibioticoterapia, termoterapia y buchadas tibias con solución salina. Se mantuvo la férula durante 21 días, logrando consolidar las fracturas y 21 en su alveolo. Se logró reimplantar el diente avulsionado evitando las secuelas estéticas, funcionales y psíquicas del trauma; la niña se reincorporó a la sociedad.


ABSTRACT Facial fractures are rare in children, for that reason the case of a 10-year-old girl is presented. She is a mestizo, urban origin, with a history of health who after a fall of approximately 2.5 m in height, suffered a severe anterior and inferior anterior dentoalveolar trauma causing a dentoalveolar fracture in the anterior superior sector from 12 to 22, dislocation of 12, 11 and 22 and avulsion of 21 (which could be recovered at the site of the trauma) and dentoalveolar fracture in the anteroinferior sector from 32 to 42. The case was received at the emergency department of the Pediatric Hospital Paquito González Cueto University and attended in the Maxillofacial Surgery room. Surgical treatment for soft tissue wounds was performed by suturing, reducing and stabilizing the fracture in the anterior superior sector by means of Erich's splint and in the anteroinferior sector by means of osteosynthesis with wire in figure-eight. Twenty first was reimplanted and fixed to the splint by osteosynthesis with wire. Treatment with soft and liquid diet, analgesics, antibiotic therapy, thermotherapy and warm mouth rinse with saline solution was indicated. The splint was maintained for 21 days, managing to consolidate the fractures and 21 in its socket. It was possible to reimplant the avulsed tooth avoiding the aesthetic, functional and psychic consequences of trauma; the girl rejoined society.
